If the record titles are identical, it's very likely
a result of having the same record open in multiple
tabs or windows at the same time. The most common
situation is opening another tab or window when
searching for new classifications to assign; after
they're assigned it goes back to the record editing
page, you end up with the same record open in
multiple windows, and the user clicks on "Add Record"
in both windows, forgetting they clicked on it the
first time or thinking that the first click didn't
work, which creates duplicate records.
We'll likely add something in the next release to
try to prevent this, though it's hard to do so
without hobbling some intended uses.
