[SPT/CWIS] CWIS 1.4.1 Released

The account activiation email message template is missing the activation
link.
Users are not aware that activation is required,  and they can't understand
why they can't log in.

To work around this I commented out the line in RequestAccountComplete that
assigns the "privilege" of disabled-account to new accounts.

The rest of account activation looks fleshed out,  just appears to be the
template from dev/test,  which is stored in the database sys config,  didn't
make the release.
